Does anyone know why the login screen looks like this (the "com_str" stuff)? Oops, can't insert a local picture.

These form fields won't accept my usual credentials. What is it asking for?
All I'm trying to do is set this dang NVR for remote access. The screen in question comes up with port forwarding set on my router and I'm using the public IP address of the router. The AmcrestDDNS method doesn't seem to work. Yet.
<addendum> the xxx.amcrestddns.com:37779 (the port mapped to HTTP inside the 5216) just returns a white screen.
<errata to addendum above> Login screen comes up with "com_str" prefixes on userid and password.
One step forward, two steps back.

I would Reset the NVR and try again. It looks like it was maybe updated and is trying to display 2 different HTML formats. Had mine do that once when I did a update and after the update when I tried to log in it acted that way.
Try a different browser, clear cache or Reset the NVR to factory

I set up a shortcut on my desktop and the "com_str" prefixes disappeared -- normal login screen. WTHeck?

I suggest you remove the port forwarding stuff, and use Amcrest P2P to remotely access your NVR from a PC using Amcrest Surveillance Pro. Camera playback should work well then. You could also use P2P to remotely access your NVR from a phone/tablet using Amcrest View Pro.
