Has anyone successfully achieved the following combo: ispy + Amcrest IP2M-841 + h264 @ 1080p/30
I'm able to get ispy + Amcrest IP2M-841 + MJPEG @ 1080p/30 working. And it appears that by leaving ispy config alone and just logging into the camera and changing it to h264 I can get it to stream to ispy, but the frame rate drops to around 15 fps. But I can't get h264 @ 1080p/30fps to work. It works fine with BlueIris but not ispy.

I have an IP2M-842 and I would love to get your ispy camera source setting. I'm getting only 0.66 FPS using the 64-bit version of ISpyConnect.
The reason your frame rate is low is because the TCP implementation in iSpy is pretty slow. You can measure this yourself using Sysinternals' Process Monitor filtering out everything but TCP Read or TCPcopy (and run only iSpy so you don't get other junk) and you will see the ISpy transfer rate is around 50% of WebActiveX when you use Internet Explorer to view the live stream.
I don't have Blue Iris but I suspect their TCP implementation is different from ISpy's.

One other detail: if you use Internet explorere to change the camera to H.264 compression and save the setting, then run ISpy, with the stream selection setting mjpg like this:
http://admin:[email protected] ... /video.cgi
then go back to internet explorer and look at the video page you will see the camera's been changed back to mjpg compression.
I sent numerous emails to Amcrest support but they are of no help. The GET (http command) statement to change from mjpg to h.264 is not documented in the user manual, and I think that's all the support staff know about.

I feel your pain. I just got my first Amcrest camera and am having a hard time with these two. But it appears to me using the URL you tried switches the camera to mjpg stream regardless of the saved configuration. Instead, try this:
rtsp://user:password@ip:port/cam/realmonitor?channel=1&subtype=0
Substitute your camera ip and rtsp port. If using the default of 554, I think you can leave off the :554 after the ip. This seems to work for me.
I created a second user account to use (instead of admin) and limited the rights to just live streaming since I have these in the URL.
But what I really want to find is the URL to view the multicast H.264 stream. I need to have 30+ clients viewing the stream, all on the local LAN, plus I want them all synchronized. So multicast is the only viable option.
But all the URLs that I try don't work...

I should have mentioned I am not using iSpy. I was just using VLC to display the stream. Using a URL similar to what you listed, the camera reverted to mjpg stream and ignored my saved configuration. Using the URL format I listed used the save H.264 configuration, at least from VLC.
I didn't mean to imply it worked for me from iSpy (which I have not used).
